MDS Coordinator

Good Samaritan Health Center seeks an MDS professional to support high-quality, person-centered care for underserved patients. In this role, you will coordinate and document assessments, organize clinical data, and support care planning to ensure services reflect patients’ physical, behavioral health, and social needs. You will collaborate with an interdisciplinary team, maintain accurate records in electronic systems, follow regulatory and payer guidelines, and help track outcomes that advance health equity in a faith-based, mission-driven environment focused on dignity and whole-person wellbeing.
