Position Summary The Care Coordinator provides direct services, case management and care coordination to an assigned caseload of individuals and families. The role helps clients access needed supports and resources in the community, facilitates ongoing communication, and advocates between the family, DCF, school, probation, and any other involved entities or service providers. The position may also facilitate or co‑facilitate groups in accordance with standard practice. Bilingual in Spanish and English required. $2,288 annual bilingual stipend! Education and Experience/Qualifications BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling, Criminal Justice or related field is required. Prior experience working with clients with mental health and/or co‑occurring substance use disorders is required. Effective time management, strong verbal and written communication skills are required. Valid driver’s license is required; the role includes vehicle operation in the community based on program needs. Submit and review documentation as defined by program. Provide front‑line crisis support to individuals and/or families by phone, in‑home, and/or in the community as needed based on initial assessment and program parameters. Problem‑solve by interpreting and adapting guidelines, reviewing situational details, and consulting with a supervisor for a second opinion. Make decisions based on interpretation of data presented and refine methods and techniques used; use judgment to adapt program guidelines and procedures to meet client/family needs. Determine processes and methods to follow based on assessment of the client/family presentation and strengths. Take action from varied alternatives to provide client/family‑centered support and care. Provide group, individual, and/or family counseling to clients with appropriate supervision, and coordinate treatment phases with internal and external providers as appropriate. Maintain records of counseling, coordination of care, child and family team meetings, and case management activities for reporting and tracking; complete all documentation for client medical records according to timelines and regulatory requirements, including Medicaid, licensing, and other agencies. Demonstrate knowledge of basic psychopathology, crisis theory, and other behavioral health concerns, and primary intervention modalities relevant to the target population (e.g., skill building, safety planning, parenting skills, high‑fidelity wraparound care). Support daily program‑specific operational functions as required. May transport clients to appointments or linkages as appropriate. Provide service (evaluation, treatment, wraparound team facilitation, follow‑up, case management) in office and community settings outside the office (homes, schools, hospitals, institutions, other community locations), maintaining a flexible schedule to ensure services are delivered as needed. Participate in development, implementation, and facilitation of initial treatment plan or plan of care, transition plan, and timeline for strategies and next steps defined by program. Demonstrate knowledge of internal and external levels of care, referral processes, and resources; make clinically appropriate referrals for identified support and service needs in a timely manner. Meet direct service productivity goals defined by program. Understand program, department, and agency guidelines, goals, and target outcomes; adhere to practice standards, data collection, and reporting protocols. Identify potential risk factors and take measures to reduce those risks in the home through supervised, trauma‑informed, relationally‑based interventions as defined by program. Attend and prepare for scheduled supervision, team meetings, staff meetings or rounds to aid coordination and review of treatment plans. Seek additional supervision or consultation as needed and follow through with directives. Communicate in a positive, effective manner with clients, families, and all internal and external providers. Continuously develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of served groups, and apply culturally responsive methods, skills, and techniques.
