Care Manager - Health Home & Community Care

CDS Rx

Prime Care Coordination in West Seneca, NY is seeking a Care Manager to partner with individuals with I/DD, their families, and providers to coordinate care and services that support health, independence, and community integration.

The role delivers Health Home core services, including comprehensive care management, care coordination, health promotion, and family support, always using a person-centered approach and maintaining HIPAA compliance.
