Concierge Home Visits | Per-Visit Opportunity

We are seeking experienced Nurse Practitioners who understand that exceptional care is not only about clinical skill. It is also about how you communicate, how you make someone feel, how you respond when plans change, and whether you take ownership of the entire patient experience.

Our providers will care for members in their homes, hotels, and other private settings. We are looking for clinicians who are confident working independently while bringing warmth, professionalism, sound judgment, and an elevated level of service to every interaction.

What You Will Do

Provide primary, preventive, and urgent care during scheduled concierge visits while creating a highly personalized member experience.

You will assess and manage acute and chronic conditions; provide patient and family education; coordinate follow-up care; recognize clinical red flags and escalate appropriately; and complete accurate, timely EMR documentation.

Based on approved competencies, services may also include IV therapy, phlebotomy, wound care, suturing, and minor procedures.

Just as importantly, you will help ensure that members feel heard, informed, respected, and cared for from the beginning of the visit through follow-up.

What We're Looking For

• Active and unrestricted New York State Nurse Practitioner and Registered Nurse licenses
• National NP certification appropriate to the position and current BLS certification
• Strong primary care and/or urgent care experience
• Excellent clinical assessment and independent decision-making skills
• Strong communication, documentation, and bedside manner
• Emotional intelligence and the ability to adapt to different personalities and environments
• Professional presence and discretion in private home settings
• A proactive, solution-oriented approach to patient care
• Reliable transportation and willingness to travel within your selected service area

Important Position Expectations

Assignments are based on patient demand, geographic coverage, provider availability, and demonstrated clinical competencies. Travel to members' homes and other approved locations is required, and visit volume is not guaranteed.

Providers are expected to respond promptly to assignments, arrive prepared and on time, follow all clinical and safety protocols, communicate proactively when circumstances change, and complete required documentation within established timeframes.
