About the Position

Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: 40 hours per week Hours per Shift: 30 minutes to 1 hour unpaid lunch Experience: Minimum 1 year in adult outpatient rehab License: Active Physical Therapist license Certifications: Current Basic Life Support certification, Valid Driver's license Must-Have: Flu vaccinations Description: Provides general Physical Therapy evaluations and treatments which impact patients' functional abilities. Oversees and directs the work of Physical Therapist Assistants and/or other designated staff members. Conducts evaluations and treatments for a wide range of ages with varying diagnoses and issues. Performs assessments, identifies therapy diagnoses, formulates therapy treatment plans with goals, and implements physical therapy treatments. Implements, supervises, and modifies therapy plans of care based on assessments to optimize patient outcomes. Reviews clinical records and case histories. Attends care conferences, regular meetings, and remains in contact with referring providers and other healthcare professionals to review patient status and individual needs. Collaborates with team members to arrange appropriate medical equipment and establish care plans prior to discharge. Documents patient encounters, including diagnoses, treatment plans, recommendations, and prognoses according to policies. Provides services in a timely manner and according to regulatory requirements. Works with both in-patient and out-patient populations in various assigned settings. Directs the work of support staff, students, or interns. Facilitates a healthcare partnership between providers, patients, and their families. Develops, implements, and conducts patient education and therapy programs based on family needs. Provides patients and families with clear and concise home exercise programs to achieve maximum rehabilitative potential. Ensures home program instructions are understood through feedback and return demonstration from patients or caregivers. Advises patients and family members on methods that foster self-care and independence. Creates home programs that are clear and ensures instructions are understood. Assists senior staff in administrative matters, clinic operations, and specialty areas as appropriate. Utilizes the computer system to obtain patient information and stay abreast of directives. Participates in the timely and efficient flow of information via digital platforms as appropriate. Provides summaries of results, statistics, and metrics as required. Assists in acquiring and fitting durable medical equipment based on patient needs. Communicates with supervisors and/or equipment providers to ensure proper measurement, fitting, and stocking of equipment. Maintains supply stocks and clinical equipment at acceptable levels. Stays current with billing procedures for reimbursement of services and compliance with providers. Maintains cleanliness of the clinical space. Educates staff on PT-related issues and educates patients and families about functional limitations and therapeutic exercises. Assists in committee and management duties and represents the Client Department as requested. Provides therapy consultation, presentations, and in-services regarding therapy services as assigned. Adheres to regulatory and practice standards of Physical Therapy. Follows all confidentiality and quality assurance standards; participates in quality management functions and utilizes principles in all aspects of the job. Performs other duties as assigned.
