St. Lukes in the Desert is seeking a compassionate organized and service-oriented Resident Wellness Manager to serve as the primary point of contact and advocate for our senior residents. This role is centered on resident wellness care coordination communication and ensuring residents receive the support and services needed to maintain quality of life and independence.

The ideal candidate is someone who enjoys building relationships with seniors coordinating services solving problems proactively and creating a welcoming supportive community environment.

Key Responsibilities

• Serve as the primary wellness and support contact for residents and families
• Coordinate and schedule medical appointments transportation and follow-up care
• Maintain regular communication with healthcare providers caregivers and family members as appropriate
• Monitor resident well-being and identify changes in needs or concerns
• Assist residents in accessing community resources and supportive services
• Support move-ins resident orientations and ongoing wellness check-ins
• Help coordinate wellness programs activities and resident engagement opportunities
• Respond to resident concerns with professionalism empathy and urgency
• Maintain accurate documentation and resident records
• Work collaboratively with community leadership and staff to ensure excellent resident care and customer service

Qualifications

• 18 years of age
• High school diploma or equivalent
• Experience working with seniors healthcare coordination social services hospitality or resident services preferred
• Strong organizational and communication skills
• Compassionate patient and service-focused approach
• Ability to manage multiple priorities and maintain confidentiality
• Bilingual English/Spanish preferred but not required
• Proficiency with basic computer systems and scheduling tools
