Northbrook Manor Care Center is seeking a Physical Therapist (PT) to join our in-house rehabilitation team . This is an excellent opportunity for a compassionate, motivated PT who enjoys working with adults in a subacute rehabilitation and skilled nursing environment and is passionate about helping residents regain strength, mobility, independence, and quality of life.

Responsibilities

• Evaluate residents' physical, functional, and mobility needs.

• Develop individualized treatment plans based on clinical findings and resident goals.

• Provide skilled physical therapy services to improve strength, balance, coordination, endurance, mobility, and functional independence.

• Treat residents recovering from surgery, illness, injury, stroke, falls, fractures, neurological conditions, and other medical conditions.

• Provide gait, transfer, balance, therapeutic exercise, and functional mobility training.

• Recommend appropriate assistive devices and educate residents on their safe use.

• Monitor resident progress and modify treatment plans as clinically appropriate.

• Educate residents, families, caregivers, and facility staff regarding mobility, safety, positioning, and fall prevention.

• Participate in care planning and interdisciplinary team meetings.

• Communicate changes in resident condition and treatment recommendations to appropriate team members.

• Complete timely and accurate clinical documentation.

• Follow applicable professional, state, federal, and facility requirements.

• Promote resident safety, dignity, independence, and quality of life.

Qualifications

• Doctorate or Master's degree in Physical Therapy from an accredited program.

• Current Iowa Physical Therapist license or eligibility for Iowa licensure.

• Current CPR/BLS certification, as required.

• Experience in skilled nursing, subacute rehabilitation, acute care, or another medically based setting is preferred but not required.

• Strong clinical, communication, organizational, and documentation skills.

• Ability to work effectively as part of an interdisciplinary team.

• Compassionate, dependable, and resident-focused approach to care.

Ideal Candidate

The ideal candidate is a patient-centered and clinically skilled Physical Therapist who enjoys helping adults achieve meaningful functional improvements. You should be comfortable working with a diverse rehabilitation caseload, collaborating with nursing and other therapy disciplines, and developing practical treatment strategies that help residents safely return to their highest level of function.
