Social Worker - Case Management

Medical Social Worker
Schedule: Shift: 5x8, Days - Weekends: Required — every other weekend
Assignment Details: Contract Length: 12 weeks - Guaranteed Hours: 72 hours per 2-week pay period
Requirements: Washington license (in-hand) - 1 year clinical experience - Acute care and discharge planning / care coordination experience - Experience with Med-Surg, LLOS, ED preferred - Required certifications: BLS - EMR: EPIC
Roles & Responsibilities
Provide clinical social work services including discharge planning and care coordination. Collaborate with an interdisciplinary team to ensure comprehensive patient care. Engage in care management, addressing a diverse range of patient needs. Facilitate access to community resources and support services for patients and families.
Compliance / Notes: Orientation: 40 hours - Compliance with requests for time off included at submittal. Must commit to working 3 of the 5 listed holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day). Must float across hospital areas and to sister campuses as needed.
