Responsibilities The Licensed Practical Nurse (LPN) is responsible for providing thoughtful care to residents and their families based on the personalized Plan of Care, monitoring and tracking changes in conditions and needs, and proactively updating the appropriate contacts on the plan of care, health changes, and concerns.
