Compassionate Hospice RN Case Manager

Gentiva Hospice

Gentiva Hospice is seeking an experienced Registered Nurse to serve as a Hospice RN / RN Case Manager. You will deliver and document skilled nursing care in patients' homes or other settings, guided by individualized plans of care, while coordinating with an interdisciplinary team. You will assess, educate, and support patients and families, ensuring symptom management and appropriate care transitions. A current RN license and hospice experience are preferred. #J-18808-Ljbffr
