Role Overview: The Physical Therapist in Home Health assesses patients' functional status, mobility, endurance, pain, and medical conditions to develop and implement personalized treatment plans. They provide interventions aimed at improving patient well-being, monitor progress, and collaborate with healthcare team members to ensure effective care. Key Responsibilities Evaluate patients' physical therapy needs and assist in developing care plans. Deliver therapy services within state legal scope and according to the plan of care. Observe and report patient responses, changes, and progress to supervising staff. Administer treatments to relieve pain, restore or maintain function. Educate patients, families, and healthcare team members about care plans and progress. Visit patients, document visits, and complete evaluations and progress notes. Develop PT plans within state guidelines and report outcomes and care frequency. Qualifications & Skills Candidates must hold a current state licensure in physical therapy, CPR certification, a valid driver’s license, and reliable transportation. They should demonstrate the ability to work effectively in diverse home environments and with varied patient populations.
