·         Provides social work/counseling/therapeutic services to our residents. 

·         Participates with interdisciplinary team members in developing and implementing program policies and procedures.

·         Assesses the social and emotional needs of patients. 

·         Assists in tracking and following up on all referrals.

·         Assists in coordination of social work services.

·         Serves as an advocate for patients' rights within the facility.

·         Available for crisis intervention social work as the need arises.

·         Attends all scheduled clinical meetings.

·         Ensures that all patients, visitors, guests, and employees receive personalized, prompt attention and are treated with receptiveness, dignity and respect. 

·         Assists to coordinate discharge planning. 

·         Assists to coordinate the transfer of patients to other floors or rooms.

·         Assists with all ancillary services including dental, optometry, psychological and podiatry services.

·         Maintains confidentiality.

·         Ensures a safe and comfortable environment.

·         Performs other duties as assigned.

 
Qualifications/Skills

·         Demonstrated critical thinking skills, problem-solving abilities and time management skills.

·         Ability to maintain patient privacy at all times

·         Desire to work in Skilled Nursing/Healthcare Environment.

·         Completed coursework related to the treatment of patients in a Skilled Nursing /Short- and Long-Term Healthcare Environment

·         Strong interpersonal skills.

·         Excellent verbal, organizational and communication skills.
