Responsibilities
- Provide direct patient care and support under supervision, including routine nursing assessments and monitoring
- Administer medications and treatments per physician orders and facility policy
- Document care activities, observations, and changes in patient condition in the charting system
- Assist residents with activities of daily living and maintain a safe, clean, and comfortable environment
- Collaborate with the interdisciplinary care team to develop and adjust care plans
- Report significant changes in patient status to the supervising nurse and respond promptly to clinical alarms

Requirements
- Active LPN license in Vermont (in good standing)
- Graduation from an accredited practical nursing program
- Relevant experience in long-term care or geriatric patient populations preferred
- Ability to work 40 hours per week including Day and/or Evening shifts
- Willingness to travel to multiple facilities within the assignment scope
- Excellent communication skills and patient-focused approach
