Family Practice/Primary Care Nurse Practitioner

Mon Fri, 8:00am 4:30pm
40 hrs/week

Role Overview

Manage patients with chronic comorbidities , primarily CKD and ESRD/ESKD
Perform annual wellness exams in home and virtually
Coordinate care with PCPs, specialists, and ancillary services
Prescribe medications, order tests, and collaborate with physicians

Requirements

2+ years of direct patient care
FNP, AGNP, or ACAGNP
Board certified (ANCC or AANP)
Active DEA
Active Georgia NP license
Nephrology experience preferred
