Registered Nurse (RN) Case Manager

The Registered Nurse Case Manager coordinates the management of care to ensure optimal utilization of resources, effective service delivery, and compliance with external review agencies. The role involves providing ongoing support and expertise through comprehensive assessments, care planning, implementation, and overall evaluation of individual patient needs. The aim is to enhance patient management quality and satisfaction while promoting continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning functions.

Key Responsibilities:

• Coordinate comprehensive assessments and care planning for individual patient needs.
• Oversee plan implementation and overall evaluation to ensure alignment with patient preferences.
• Enhance the quality of patient management through effective collaboration within the interdisciplinary team (IDT).
• Act as a patient advocate to address holistic needs and resource utilization effectively.
• Facilitate discharge planning and ensure smooth transitions across acute care settings.
• Monitor compliance with external review agencies and ensure adherence to best practices.

Qualifications:

• Current CA RN license.
• Basic Life Support (BLS) certification through the American Heart Association.
• A minimum of 2 years of recent experience in RN Case Management.
• Experience in Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) settings.
• Familiarity with the MCG guidelines and discharge planning across acute care settings is preferred.
• Strong interdisciplinary team collaboration skills.

Work Schedule:

This position primarily operates Monday through Friday, with standard working hours.
