Infectious Disease-Clinical Pharmacy Specialist Ambulatory

Infectious Disease Clinical Pharmacy Specialist – Ambulatory Care As a member of Community Health Network’s ambulatory care pharmacy team, you will expand pharmacy services within Infectious Disease Clinics. Your role will involve close collaboration with clinicians and patients to ensure optimal medication therapies. Responsibilities Collaborate with Infectious Disease (ID) physicians for optimal HIV, PreP, and Hepatitis C medication selection and monitoring. Participate in managing patients receiving outpatient parenteral antimicrobial therapy (OPAT). Aid in ambulatory antimicrobial stewardship efforts and serve as a resource for ID‑related drug information questions. Take a leadership role in staff education, guideline and policy development, implementation, and performance‑improvement initiatives. Mentor and teach pharmacy students and pharmacy residents, and serve as a clinical resource for pharmacy staff. Essential Skills and Qualifications Strong communication skills and a collaborative approach to patient care. Ability to work closely with physicians and health‑care teams while providing direct patient care. Experience with a collaborative drug‑therapy agreement, meeting with patients to provide extensive drug and disease‑state education focused on HepatitisC, HIV, PrEP, and OPAT. Highly motivated to expand infectious disease pharmacy services as appropriate. Ability to work closely with patients to overcome medication adherence issues. Assistance with medication‑assistance coordinators to help patients obtain cost‑effective medications. Ability to serve as a drug‑information resource for other providers and staff, answering medication‑therapy questions. Ability to monitor for drug‑therapy‑related adverse events by performing or ordering appropriate clinical or laboratory tests. Engagement in population‑health projects to impact the lives of patients served. Clear understanding of quality‑improvement principles. Ability to precept pharmacy students and residents. Graduate of an accredited college of pharmacy (Bachelor or PharmD); Doctorate degree. Indiana Pharmacist license. Successful completion of a PGY1 Pharmacy Residency and a PGY2 Infectious Diseases Pharmacy Residency or HIV Residency, OR a minimum of five (5) years equivalent experience in an ambulatory infectious disease setting. Experience working under a drug‑therapy agreement in a clinical setting is strongly encouraged. Board certification as a Board Certified Infectious Diseases Pharmacist, Board Certified Pharmacotherapy Specialist, or Board Certified Ambulatory Care Pharmacist required within two years of accepting the position.
