What You'll Do:

• Supervise and guide a dedicated team of Nurse Practitioners, ensuring high standards of care are met across nursing homes and PACE sites.
  
  
• Perform comprehensive annual wellness assessments, adapting care plans for participants with complex needs, frailty, or higher risks.
  
  
• Be the go-to expert for urgent care, same-day assessments, and seamless transitions of care for participants returning from the hospital.
  
  
• Work hand-in-hand with Primary Care Providers to co-manage care for high-risk participants, always seeking ways to fill care gaps.
  
  
• Lead home care coordination, creating personalized care plans that promote health and independence, with ongoing follow-up for the best possible outcomes.

What You Bring:

• A minimum of 3 years of clinical nursing experience (preferably in home care or care management).
  
  
• Strong background in geriatrics, internal medicine, or primary care with experience working with elderly or frail populations.
  
  
• A valid NY State Nurse Practitioner license, DEA Certification, and AANP or ANCC Board Certification.
  
  
• Comfortable providing home care 20%-50% of the time.
