- Position Type: Contract
- Contract Duration: 3 months
- Start: June 29, 2026
- Pay Rate: $70.00 / Hour (USD)
- Shift / Schedule: Day shift, 5x8 Hours, 8:00 AM - 4:30 PM. *This facility cannot accommodate 4x10s or 3x12s.*
- Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
- Travel Requirements: Traveler is expected to have their own transportation. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day. Mileage Reimbursement: IRS Standard.
- Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.
- May visit up to 6 patients per day; 30 productivity points standard (1:1 ratio)
- Complete Oasis assessments (required)
- Chart using EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/Oasis
- Oasis documentation required
- Own transportation required
- Ability to manage a home health caseload with up to 6 patient visits per day
- PT License required
- BCLS certification required
- OR state physical therapy license required (pending license accepted)
- Minimum 1 year of Skilled Home Health experience required
- Home health setting - Seaside, OR and surrounding areas
- Occasional maximum drive radius of roughly 60 miles (60-90 minute total drive time) per day
- Business casual attire; no jeans; scrubs allowed but not required (no specific color)
- No weekend requirement
- No on-call requirement
- No floating between units required
- Block schedule not available
- Locals accepted; considered "local" within 50-75 mile radius; locals accepted at the same rate
- Parking: Free
- COVID vaccination requirement: Primary series plus one booster required
- RTO Restrictions: Confirm with hiring manager
- First-time travelers accepted
