Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking the following healthcare professional to join our team, supporting our Monticello, Spring Valley, Haverstraw and Peekskill health centers. We currently have the need for a Nutritionist at our Amenia and Peekskill locations.The Dietitian will perform individual counseling of patients referred by the medical personnel in all sites of Hudson River Healthcare. Performs job duties in compliance with systems and job specific standards.Essential Job Duties and Responsibilities:Provides nutrition counseling to patients referred by cliniciansPerforms initial nutrition history on each patientDevelops and implements a care plan based on the assessment of nutritional and health needsProvides instructions on a normal or special dietSchedules follow-up appointments and evaluates the reaching of goalsProvides nutrition counseling for diabetic patients as part of diabetic care planProvides nutritional education materials to patients to support counseling effortsMakes referrals to other health care providers and community services as appropriateCounsels individuals and families on nutrition and diet based on the particular needs of the individual or family as determined by their nutritional and health riskAttends staff meetings and in-service training sessions at both local and state levelsProvides consultation into the development of program policies and proceduresProvides direction and executes health education programs as neededMust demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients servedMust demonstrate knowledge of the principles of grown and development over the life spanMinimum Education Requirement: Bachelor's DegreeRelated Work Experience: 2-4 yearsPreferred Education Requirement: Master's DegreePreferred Work Experience: 5-8 yearsRequired Licensure: RD/ RDN and CDN with a valid CPR cardSpecialized Skills & Knowledge:Ability to communicate in English, both orally and in writingAbility to operate a motor vehicle and travel within the HRHC service site areaKnowledge of nutrition principles as well as knowledge of nutrition during pregnancy and lactation, as well as infant and child feeding practicesKnowledge of the geographic community serviced and resourcesCompetence in nutrition assessment counselingAbility to collaborate with other member of Hudson River HealthCare staff in order to maintain high quality standards and achieve client satisfactionSalary Range: $23 - $27 per hour
