In this role, you will be a vital anchor for our healthcare team, assessing and monitoring all aspects of our individuals' medical and psychiatric care. What You'll Do (Responsibilities)

• Collaborative Care: Partner with community healthcare providers, nutrition services, and behavior specialists to deliver high-quality, holistic care.
• Care Coordination: Oversee and coordinate all healthcare appointments, ensuring thorough and timely follow-ups.
• Team Education: Train, empower, and delegate medication administration and nursing tasks to the Direct Support Team in accordance with state licensing and Mosaic policies.
• Medical Record Management: Maintain, review, and audit medical records to monitor health concerns and document the delegation of nursing tasks.

Schedule & On-Call Structure

• Primary Hours: Generally day hours.
• On-Call Rotation: Every 3rd week (primarily phone-based for emergencies; in-person response is rare).
• On-Call Support: You are never on an island—you will be backed by a strong support network, including a team of supervisors, QIDPs (Qs), and an RN Supervisor.

Schedule: Generally day hours Monday-Friday some on call responsibilities (mainly via phone)/flexible hours

• Bachelors of Science in Nursing, or the degree required based on State and agency definitions.
• Minimum of two years of general nursing experience required.
• State nursing license required.
• Work requires frequent physical activity including extended periods of standing, walking, and bending with occasional periods of sitting, kneeling, climbing, stooping, crouching, squatting and balancing.
• Work also requires constant reaching between knee and shoulder level as well as frequent reaching below knee level and overhead.
• Work requires occasional independent lifting up to 25 pounds, frequent push/pull up to 40 pounds of force and the ability to safely transfer 50 pounds.
