About This Role The RN/Case Manager will oversee all aspects of case management for assigned patients, ensuring appropriate levels of care and collaborating with an interdisciplinary team to enhance patient outcomes. Highlights Pay: $63.28/hour Location: New York, NY Schedule: Full-time, Day shift What You'll Do Review new admissions to identify needs for utilization review and discharge planning. Perform continued stay reviews to justify inpatient stays. Collaborate with physicians to expedite diagnostic testing and treatment. Document all clinical reviews in the Case Management Documentation system. Assess patients' clinical, psychosocial, and functional status with the interdisciplinary team. Identify variances affecting length of stay and discharge planning. Conduct follow‑ups on treatment delays and reporting of results. Develop specific goals with the physician and interdisciplinary team.
