Requirements
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities
- Perform clinical assessments to identify patient needs and appropriateness for care.
- Document, prepare, and review patient care plans and progress notes, revising as necessary based on ongoing assessment data.
- Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
- Assist and instruct patients and families in safe transfer techniques and proper use of equipment.
- Provide education to patients and families about therapeutic procedures considering their age, condition, culture, and learning readiness.
- Apply infection control concepts and universal precautions in patient care activities.
