We are seeking an experienced Nurse Practitioner to join our primary care practice. This a full-time position providing the opportunity to care for patients ages 14 and older in a fast-paced, professional environment. The ideal candidate is confident evaluating, diagnosing, and treating a wide range of acute and chronic conditions while delivering high-quality patient care. Responsibilities

• Provide comprehensive primary care services, including independent evaluation, diagnosis, and treatment of patients ages 14 and older.
• Conduct preventive, wellness, chronic care, and acute sick visit assessments.
• Develop, implement, and manage individualized treatment plans based on evidence-based clinical practices.
• Order, review, and interpret laboratory tests, imaging studies, and other diagnostic procedures.
• Communicate test results, treatment recommendations, and follow-up plans clearly and compassionately to patients.
• Educate patients and families on medical conditions, preventive care, medications, and healthy lifestyle practices.
• Coordinate patient care by initiating referrals to specialists and other healthcare resources when appropriate.
• Maintain accurate, thorough, and timely documentation within the electronic health record (EHR) system.
• Prescribe medications and therapies in accordance with state regulations and organizational policies.
• Monitor and manage chronic conditions, ensuring continuity of care and positive patient outcomes.
• Collaborate effectively with physicians, advanced practice providers, nurses, medical assistants, and administrative staff to deliver high-quality, patient-centered care.
• Support quality improvement initiatives and contribute to efforts that improve clinical outcomes, patient satisfaction, and practice efficiency.
