LPN- Full Time

Licensed Practical NurseThis position will float between both of our Tensas Rural Health Clinics, as needed, which includes the Newellton and St. Joseph Rural Health Clinics. This is a full time position (40 hours per week) with benefits. This position's schedule will be Monday - Friday (7:30a - 5:00p or 8:30a - 5:00p** depending on clinic location) and will be included on the Saturday rotation for the Newellton RHC which is from 7:30a-12p. Directly responsible for performing patient care functions according to established standards, policies and procedures.Job Relationships: Responsible to the Rural Health Clinic Director. We are committed to delivering compassionate, capable, and personalized treatment to our patients and their families, always keeping in mind that our responsibility is to the health needs of the people.Essential Job Functions:Obtain and record vital signs, assessments, chief complaints, medical history, height/weight, etc. as appropriatePerform basic clinical procedures such as wound dressing, specimen collection, point of care testing, EKGs, etc. (as trained)Administer and record all medications and treatments based on provider orders accurately using proper proceduresRecord all care information concisely, accurately, and completely in a timely mannerAssist providers with exams, minor procedures, and urgent care needsPrepare, stock, and sterilize exam rooms and clinical equipmentCommunicate test instructions and follow-up plans to patients clearly and effectivelyAssist with patient outreach for lab results, chronic disease follow-up, and preventative-care remindersCollaborate with the care team to ensure timely documentation and quality care deliveryAccurately document all patient interactions, procedures, medications, and communications in the Electronic Health RecordFollow RHC regulations, safety protocols, and infection control standardsMonitor and maintain clinic supplies and equipment usage. Notifies appropriate departments to replenish stock, as necessary.Participate in Performance Improvement on a monthly basis.Maintain knowledge regarding use of defibrillators, holter monitors, etc.Perform and documents quality controls dailyUphold HIPAA and patient privacy standardsProvide basic health education, medication instructions, and lifestyle guidelines within skill level and trainingMaintains a neat and orderly work area.Assists with diabetic foot care, as neededPerforms other duties as assigned.Qualifications:Education and Training: Graduate of a course for LPN. Must be licensed in the state of Louisiana. CPR certified. Work Experience: No experience necessary.
