We embrace Orange County's diverse communities, educate families and individuals about available resources, and empower them to improve their health, stability, and quality of life through culturally responsive, collaborative services.Position SummaryAbrazar is seeking a bilingual, relationship-centered Home Visitors Case Manager to provide evidence-based home visiting and case-management services to families with young children throughout Orange County. The Case Manager establishes consistent, long-term relationships with participating families, primarily through in-home visits, and uses a family-centered, strength-based approach to support healthy child development, positive parent-child relationships, family stability, and overall well-being.This classification is designed to support both the Healthy Families America (HFA) and Parents as Teachers (PAT) home-visiting models so Abrazar can maintain one strong, consistent position standard across the Home Visiting Program. The current opening is primarily assigned to HFA. Depending on program needs, training, and assignment, the Case Manager may also use PAT curriculum and practices.The Case Manager maintains an assigned caseload, completes regular home visits, conducts required screenings and assessments, partners with families to identify goals, develops and follows family service plans, connects families to health care and community resources, documents services accurately and on time, participates in reflective supervision, and collaborates with hospitals, Orange County Social Services Agency, and other Home Visiting Collaborative partners. The position requires sound judgment, strong follow-through, cultural humility, and the ability to work independently in family homes while remaining closely connected to the Home Visiting team.Compensation and Benefits$24.00-$26.00/hour: Candidates meeting the minimum qualifications with relevant experience supporting children and families, community outreach, family services, early childhood programs, benefits navigation, or comparable direct-service work.$26.01-$28.00/hour: Candidates with demonstrated experience in home visiting, case management, parenting education, developmental screening, family goal planning, community-resource navigation, or comparable family-support services, including experience maintaining an active caseload and timely documentation.$28.01-$30.00/hour: Candidates with advanced directly relevant experience, including substantial HFA, PAT, or comparable evidence-based home-visiting experience; a related bachelor's degree; strong assessment and family-support skills; complex case coordination; high-quality documentation; or other highly relevant preferred qualifications.Vacation: 12 days per year, accrued monthly after three months.Health leave: Up to 56 hours per fiscal year after three months.Holiday pay: 10 days per year after three months.Health, vision, dental, and life insurance.Essential ResponsibilitiesHome Visiting and Family SupportMaintain an assigned caseload of approximately 10-12 families referred through participating hospitals, Orange County Social Services Agency, or other approved referral pathways.Provide up to approximately 12 home visits per week and maintain regular, long-term contact with participating families in accordance with program and model expectations.Conduct home visits primarily in family homes, with virtual visits used when appropriate and permitted by program requirements.Build trusting, professional relationships with families using family-centered, strength-based, culturally responsive, and trauma-informed engagement practices.Teach and model parenting skills and support parents and caregivers in strengthening positive parent-child relationships.Provide developmentally appropriate education and guidance related to prenatal and postpartum well-being, infant and child development, parenting, family routines, and protective factors.Partner with families to identify priorities, establish meaningful goals, develop family service plans, and monitor progress over time.Support families in addressing barriers that affect stability, parenting, health, child development, or access to services while maintaining appropriate professional boundaries.Assessment, Screening, and Child DevelopmentAdminister required family assessments and screening tools in accordance with program timelines, training, and model requirements, including tools such as the FROG Scale, ASQ, ASQ-SE, depression screenings, and health screenings as assigned.Use assessment and screening results to identify strengths, concerns, developmental needs, and appropriate follow-up actions with families.Support families in understanding developmental milestones and encourage age-appropriate parent-child interaction, learning, and healthy development.Coordinate referrals and follow-up when screenings identify needs requiring medical, developmental, behavioral-health, social-service, or other specialized support.Document assessment results, referrals, follow-up actions, family goals, and progress accurately and within required timelines.Case Management, Referrals, and Family StabilityHelp families identify and access appropriate community resources, health care, public benefits, developmental services, parenting supports, food assistance, housing resources, and other services based on individual needs.Provide case-management support that strengthens family protective factors, reduces barriers, and promotes long-term family stability and self- sufficiency.Maintain positive working relationships with Orange County Social Services Agency and assist eligible families with CalWORKs-related navigation or applications when appropriate.Coordinate with hospitals, health care providers, public agencies, community organizations, and Home Visiting Collaborative partners to support continuity of services.Follow up on referrals and help families overcome practical barriers to accessing recommended services.Documentation, Model Fidelity, and QualityMaintain complete, accurate, timely, and confidential case files, home-visit notes, family plans, screenings, referrals, outcomes, and required program records.Enter and maintain required information in Visit Tracker, Bridges Connect, and other approved data or case-management systems.Follow Healthy Families America (HFA) model guidelines, best-practice standards, and required documentation expectations for assigned HFA families.Use Parents as Teachers (PAT) curriculum and practices when assigned or appropriate based on program needs and training.Maintain confidentiality and protect personal and health information in accordance with HIPAA, agency policies, and applicable program requirements.Monitor caseload responsibilities, required visit frequency, screening timelines, documentation deadlines, referrals, and family follow-up to ensure services remain current and compliant.Participate in required data review, quality-improvement activities, program monitoring, audits, and corrective follow-up when needed.Reflective Supervision, Training, and Team CollaborationParticipate consistently in weekly reflective supervision and use supervision to strengthen family engagement, professional judgment, case planning, and service quality.Complete HFA, PAT, agency, safety, confidentiality, screening, and other required training within established timelines.Participate in ongoing professional development, case consultation, team meetings, and collaborative learning activities throughout the program year.Communicate promptly with the Home Visiting Program Director regarding significant family concerns, service barriers, missed contacts, safety issues, or situations requiring additional guidance.Work collaboratively with Home Visiting team members and other Abrazar programs to coordinate resources and strengthen services for families.Community Outreach and PartnershipsAttend community events, outreach activities, and partner meetings as assigned to connect eligible families with home-visiting and other supportive services.Develop and maintain effective working relationships with participating hospitals, Orange County Social Services Agency, collaborative partners, and community-based organizations.Represent Abrazar professionally and communicate home-visiting services accurately, respectfully, and in culturally and linguistically appropriate ways.Minimum QualificationsBilingual proficiency in English and Spanish or English and Vietnamese.High school diploma or equivalent.Relevant experience working with or providing services to children, parents, caregivers, or families.Experience working effectively with culturally and linguistically diverse families and communities.Working knowledge of infant and child development and the ability to communicate developmental information clearly to parents and caregivers.Strong organization, follow-through, time management, verbal communication, written communication, and interpersonal skills.Ability to maintain an active caseload, schedule and complete recurring family contacts, meet documentation deadlines, and work independently in community settings.Ability to build trusting relationships while maintaining professional boundaries, confidentiality, and sound judgment.Proficiency with Microsoft Office, including Word, Excel, PowerPoint, and Teams, and ability to learn electronic case-management and reporting systems.Reliable transportation, a valid driver's license, appropriate automobile insurance for any personal vehicle used for work, and ability to qualify for Abrazar's insurance coverage.Ability to work occasional evenings or weekends when family, outreach, training, or program needs require.Preferred QualificationsBachelor 's degree in Child Development, Early Childhood Education, Social Work, Human Services, Public Health, Family Studies, Psychology, or a related field.Direct experience with Healthy Families America (HFA), Parents as Teachers (PAT), or another evidence-based home-visiting model.Experience providing home-based services, case management, parenting education, or family support to families with infants and young children.Experience administering developmental, social-emotional, depression, family-strengths, or health screening tools such as ASQ, ASQ-SE, FROG, or comparable instruments.Experience developing family goals or service plans and monitoring progress over time.Experience with Visit Tracker, Bridges Connect, or comparable home-visiting, case-management, or client-data systems.Experience working in Garden Grove, Santa Ana, Westminster, Anaheim, or with Latino, Vietnamese, immigrant, low-income, and families with young children.Knowledge of Orange County family-support, developmental, health care, public-benefit, and community resources.Knowledge, Skills, and AbilitiesAbility to establish and sustain trusting relationships with families while respecting family culture, structure, parenting practices, and individual goals.Strong understanding of strengths-based family support, protective factors, child development, parent-child interaction, and culturally responsive practice.Ability to conduct structured visits, balance relationship-building with required program activities, and keep families engaged over an extended period of service.Sound judgment and ability to recognize concerns, seek supervision appropriately, and coordinate timely referrals or follow-up.Strong observation, assessment, documentation, case-planning, and resource-navigation skills.Ability to manage competing priorities, travel throughout Orange County, maintain reliable scheduling, and complete work with limited direct supervision.Commitment to accurate documentation, confidentiality, evidence-based practice, measurable outcomes, and Abrazar's mission and values.Mandatory Employment and Driving RequirementsVerification of authorization to work in the United States.Successful completion of required tuberculosis screening, background screening, Live Scan, and drug screening as conditions of hire.Current CPR/First Aid certification or willingness and ability to obtain required certification.Reliable transportation, a valid driver's license, appropriate automobile insurance for any personal vehicle used for work, and ability to qualify for Abrazar's insurance coverage.Physical, Mental, and Environmental DemandsThe employee must be able to work effectively in family homes, office, community, training, meeting, and outreach settings; sit or stand for extended periods; travel throughout Orange County; drive to family and partner transport a laptop, files, screening materials, curriculum, and routine program supplies. The position may involve walking on uneven surfaces, bending, reaching, carrying routine materials, and exposure to outdoor weather and varied home or community environments.
