Primary Care Physician Overview Accountable for outcomes, strong clinical care, and cost‑effectiveness for a panel of up to 450 seniors. Acts as a health coach, assisting patients in setting and achieving short‑ and long‑term health goals and following up regularly. Simplifies and prioritizes care to make behavior change actionable for patients and for the care team. Key performance metrics include patient admissions per thousand, CGCAHPS (patient experience), clinical gap closures, and medical cost measures. The PCP will have specific targets for these metrics and will work toward them with center and market leadership. Essential Job Duties / Responsibilities Independently provide care for acute and chronic illnesses in older adults. Take full accountability for patient care and outcomes, seeking specialist consultation as needed while remaining involved in detailed care. Engage with the hospitalist whenever a patient is hospitalized, regardless of hospital affiliation. Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document patients with acute and complex chronic health needs. Lead the care team (medical assistant, care facilitator, and care coordinator) for patients able to attend the office. For patients who cannot come to the office, collaborate with the transitional care team, case managers, and other regional resources. Play an active role in center management, cover for other providers when needed, recruit patients and additional providers for the company. Perform additional duties as assigned and modified by management. Physician leadership: continuously develop leadership skills for patients, team, center, and company. Quality focus: work with teams to achieve high‑quality outcomes and improve practice operations. Influence: competently and compassionately influence patients, teams, and self to achieve best outcomes. Self‑care: maintain physical, mental, social, and spiritual well‑being to provide optimal patient care. Proficiency in Microsoft Office Suite and other relevant software. Willingness to travel locally and occasionally nationally for training and recruiting. Fluency in English; additional language requirements disclosed during application if applicable. Exercise independent judgment in patient care as required. Education and Experience Criteria MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty. Current, active State Medical License for the state(s) where the position is located. Board certification or eligibility in the specialty is preferred; board maintenance required. Valid DEA number for Schedule II–V controlled substances. Basic Life Support certification within first 90 days of employment. Pay Range $214,700 – $306,714 per year.
