Registered Nurse (RN) / Licensed Practical Nurse (LPN)

Our client is seeking s compassionate and patient-focused Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join their growing outpatient care team. Supporting two providers, you'll play an essential role in delivering high-quality patient care through education, care coordination, and clinical support. Responsibilities

• Support two providers with daily clinical operations.
• Conduct routine nursing visits and patient assessments.
• Provide patient education regarding medications, treatment plans, and disease management.
• Coordinate referrals and follow up on laboratory results.
• Process prescription refill requests and prior authorizations.
• Administer medication injections and other prescribed treatments.
• Perform basic telephone and in-office triage.
• Document patient care accurately within the electronic medical record (eClinicalWorks).
• Collaborate with providers and interdisciplinary staff to ensure exceptional patient care.
• Float to another clinic location on rare occasions when additional support is needed.

Qualifications


Required

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
• Strong critical thinking and clinical decision-making skills.
• Excellent communication and patient education abilities.
• Patient-focused mindset with a compassionate approach to care.
• Comfortable caring for a diverse patient population across the lifespan, from pediatrics to geriatrics.
• Ability to take initiative and work collaboratively within a team.

Preferred

• Outpatient or primary care experience.
• Infusion experience is a plus but not required.
• Pediatric experience is preferred.
• Bilingual skills are a plus.

Schedule

• Monday-Friday
• Flexible daytime schedule:
  • 8:00 AM-5:00 PM
  • 8:00 AM-4:30 PM
  • 8:30 AM-5:00 PM
• Consistent weekday hours with excellent work-life balance.
