We are committed to being responsible stewards of our resources, ensuring efficient and cost-effective care for all.Position Summary:The APRN at River Valley Counseling Center (RVCC) is responsible for coordinating and providing high-quality psychiatric and behavioral health care while ensuring clients are treated with dignity, respect, and confidentiality. The APRN also serves as a clinical resource for staff by providing training, orientation, peer support, and guidance on medication management. Additional responsibilities include participating in quality improvement initiatives, maintaining professional licensure and certifications, complying with RVCC policies and regulatory requirements, and working collaboratively across multiple RVCC locations. The position offers opportunities for professional growth within a supportive, team-oriented environment while maintaining client dignity, autonomy, and confidentiality.Key Responsibilities:To clinically coordinate client care, including, but not necessarily limited to, the following:To review individual client treatment plans and clinical documentation periodically to assure that client care requirements are met.To collaborate with physicians to ensure effective communication regarding medications and patient treatment plans.To act as a resource person for staff for clinical support as well as direction, to provide peer supervision to those individuals assigned by the Program Director and to establish as well as run an APRN support.To communicate regularly and clearly with clients, families, physicians, and other health care team members to assure continuity of care.To identify crisis situations and respond appropriatelyTo distinguish between normal and abnormal findings and to evaluate what findings need collaboration or referral to appropriate allied professionals or community agencies.To develop a plan of care based on identified needs and problems and to provide follow-up on clients treated, as may be necessary.To assist in identifying potential safety/health risks of individual clients and ensure appropriate and timely counseling.To conduct case management reviews with multi-disciplinary team and community agencies, as necessary, to participate in or contribute to continuing client care plans involving clients managed by outside providers.To initiate and/or complete required documentation in a timely, systematic, accurate form; to include age related assessment of biophysical, psychosocial, environmental, self-care, age-specific, and educational factorsTo competently provide care to clients, including, but not limited to the following:To explain the procedures or treatment plans according to the age and level of comprehension of each client.To provide care in a manner that respects the adult client's dignity, autonomy, and independent decision-making.To include, when appropriate, assessment data from the client's significant other/responsible person.To review assessment data obtained by other clinical disciplines, including diagnostics.To promptly reassess needs when warranted by the client's condition.To assess substance abuse issues and history, including acute withdrawal symptoms.To assess risk of suicide and dangerousness to others.To assess mental status.To act as a client advocate to ensure that clients'/families'/significant other's needs are incorporated into the plan of care.To develop/initiate or assist in developing/initiating the treatment plan for each assigned client with appropriate timeframes, in conjunction with the multi-disciplinary treatment team.To involve the client/significant other/health care agent in planning care as appropriate.To include discharge planning needs on multi-disciplinary treatment plans.To provide treatment interventions that are age-specific, as designated in the treatment plan, to meet the client's/family's or significant other's needs.To provide education to client/family/significant other/health care agent specific to client's healthcare needs and level of comprehension.To implement physician's orders accurately and within expected time frame.To take a leadership role in supporting clinical staff in interventions with clients in crisis, clients in need of higher level care (e.g. medical detox), or clients who appear not to be benefiting from treatment.To facilitate groups, as needed.To participate actively in the quality improvement process and incorporate quality improvement findings into clinical proactive and coordination of client care in the department. To also be involved in any administrative meetings as a representative of the medical staff.To coordinate and provide or assist in providing orientation for new nursing and/or clinical staff.To review and reorder medical supplies within approved budgetary limits.To keep current with public and mental health licensing and regulatory requirements regarding medication clinics.To update appropriate RVCC staff, as necessary, about proper regulatory procedures for medication clinics.To develop and maintain proficiency in using RVCC's computerized client information systems accurately and effectively.To be proficient with the use of RVCC's interpretive services access appropriate service(s) to effectively meet the needs of the clients being served.To work at multiple sites serviced by RVCC, as assigned or directed.To utilize tactful, direct, and sensitive interaction skills in promoting positive customer relations and interpersonal relationships at all levels within and outside the organization. To represent RVCC, both within and outside the organization, in a positive, professional manner.To communicate effectively with clients with various cultural backgrounds.To encourage questions in a friendly, helpful manner and to answer in clear, age-appropriate language.To avoid stereotyping people.To treat each person as an individual.To collaborate with immediate supervisor in identifying own strengths and weaknesses in an on-going evaluation process, to keep supervisor informed of activities within area of responsibility and to communicate needs on a timely and routine basis.To maintain and improve current knowledge through self-directed study and/or participation in on-going professional development programsTo maintain own license in current status, as required by law, retain a copy of such license on premisesTo use time constructively and organize work for maximum productivity.Qualifications:Required:Current Massachusetts Registered Nurse (RN) License (issued by the Massachusetts Board of Registration in Nursing)National APRN Certification in the appropriate specialty (e.g., Psychiatric-Mental Health Nurse Practitioner through the ANCC)Massachusetts APRN Authorization (recognition as an APRN by the Board of Registration in Nursing)Massachusetts Controlled Substances Registration (MCSR) issued by the Massachusetts Department of Public HealthFederal DEA Registration to prescribe controlled substancesPreferred:Five or more years' experience as a clinical nurse specialist; experience working with chronically mentally ill populations required, experience or desire to work with pediatric patients and bilingual (Spanish/English) ability a plus.What We Offer:A supportive, mission-driven work environmentOpportunities for professional development and continuing educationCompetitive benefits packageCollaboration with a compassionate, multidisciplinary teamThe chance to make a real difference in our communitiesSummary of Benefits:Salary Range: $187,200-$208,000Based on years of experience, level of education, specialty and licensure/certifications.
