Job Description

Position: LPN (Contract-to-Perm)

Location: NY 14527
Contract Length: 05/12/2026 – 08/15/2026 (~13 weeks )
Pay: $1,326 – $1,495/week
Schedule: Part-time, 2–3x8-hour variable Day/Evening shifts (e.g., 7:00 AM – 3:00 PM or 3:00 PM – 11:00 PM)
Hours: ~24 hours/week
Additional Requirements: Every other weekend (EOW) and holidays required

Position Summary & Responsibilities

Provide direct/indirect care to adult & geriatric residents under RN supervision
Monitor/report patient status and assist with care planning
Support ADLs, safety, nutrition, and restorative care per plan of care
Collaborate with interdisciplinary team and maintain accurate documentation
Ensure compliance with safety, infection control, and care standards

Requirements

Active NY LPN License (required)
BLS certification (AHA or RQI required)
Minimum 1 year LPN experience (LTC preferred)
High School Diploma or GED required
Experience with adult and geriatric populations
Strong communication and teamwork skills
Ability to work variable shifts, weekends, and holidays
Comfortable with potential exposure to blood/body fluids
