Physical Therapist Full Time Days Continuing Care Center Join to apply for the Physical Therapist Full Time Days Continuing Care Center role at Desert Care Network At Hi-Desert Medical Center (HDMC) we've been caring for our community for over 40 years. Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests. The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms: Subacute care: The highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care. Skilled nursing care: We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures. Long-Term Care: We provide long-term care to residents who require assistance with personal and medical needs. Position Summary Plans and implements specific treatment programs for individual patients/residents according to the principles and practices of physical therapy. $40.89 - $63.37 hourly. Position Specifications Physical Demands May be exposed to infectious and contagious diseases, body fluids and wastes. Heavy physical effort, including lift/carry up to 50 lbs. Prolonged standing/walking, bending and squatting. Must be able to lift, position and transfer patients/residents and equipment. Working Conditions Handles Emergency or Crisis Situations. Minimum Education/Certification/Experience Current California Physical Therapy License. Current BLS CPR certification. Shift: Days Hours Tenet complies with federal, state, and/or local laws regarding mandatory vaccination of its workforce. Primary Duties Evaluates assigned patients/residents and develops treatment plans, including specific goals and activities, reevaluating and modifying plans as necessary. Ensures treatments and care is done appropriately and completely in accordance with patients/residents clinical condition. Provides instructions in therapeutic procedures to be continued by the patient as prescribed by a physician. Provides direct physical therapy to patients/residents. Communicates clearly and tactfully with patient/resident regarding progress, problems, home programs and other issues related to the therapeutic process. Communicates observations, facts and comments with interdisciplinary team members individually, and through team meetings as appropriate and required. Assesses patient’s progress weekly or as required and adjusts program as appropriate. Prepares written documentation as requires by regulations and departmental policy, such as evaluations, individualized treatment plans, progress notes and other required reports. Provides direct Physical Therapy to patients per plan pf care. Maintains established departmental policies, procedures, objectives, and quality assurance.
