Hospice Registered Nurse

Location: San Antonio, TXJob Summary:The Hospice Registered Nurse (RN) is responsible for providing skilled nursing care to hospice patients in their place of residence and coordinating patient care with the Interdisciplinary Team (IDT), physicians, patients, and families. The RN performs comprehensive assessments, develops and manages hospice plans of care, and ensures quality patient care and regulatory compliance throughout the hospice care process.This role requires strong clinical assessment skills, care coordination experience, and the ability to work independently in a hospice field setting while collaborating with the interdisciplinary care team. Bilingual proficiency in English and Spanish is a significant plus.Qualifications:Registered Nurse (RN), licensed per state requirementsHospice experience preferredHome Care Home Base (HCHB) experience preferredStrong clinical assessment, communication, and documentation skillsExperience working within an interdisciplinary healthcare teamKnowledge of hospice regulations, documentation, and care planning preferredReliable transportation, valid driver’s license, and current auto liability insuranceAbility to read, write, and communicate effectively in EnglishEssential Functions:Perform initial and ongoing comprehensive nursing assessments for hospice patientsDevelop, implement, and update individualized hospice plans of care to ensure quality and continuity of careCoordinate patient care with the Interdisciplinary Team (IDT), physicians, patients, and familiesEducate patients and families on disease process, medications, symptom management, and end-of-life careDetermine scope and frequency of nursing services based on patient condition and care needsSupervise and provide clinical direction to Hospice Aides and LVN/LPN staffMaintain accurate and timely clinical documentation in the electronic medical recordCommunicate changes in patient condition to physicians and interdisciplinary team membersEnsure compliance with Medicare hospice regulations, documentation standards, and agency policiesParticipate in case conferences, IDT meetings, and care coordination activitiesMonitor assigned cases to ensure compliance with third-party payer and hospice requirementsPromote hospice philosophy and provide compassionate, patient-centered end-of-life careWe are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.Diversity, Equity, and Inclusion are at the heart of Cantex.
