Desired Qualifications:

• Bachelor's degree required; Master's or Doctorate in Physical Therapy preferred
• Active PT license in the state of Massachusetts (or eligibility)
• Basic Life Support (BLS) certification preferred
• Skilled Nursing or long-term care experience required
• Proficient in Microsoft Office and electronic medical records
• Excellent communication, analytical, and documentation skills
• Compassionate, team-oriented, and resident-focused approach
• Ability to lift up to 50 pounds and meet physical demands

Responsibilities:

• Perform comprehensive evaluations and establish individualized therapy plans
• Collaborate with residents, caregivers, and interdisciplinary teams to maximize functional independence
• Document all interactions, therapies, and recommendations accurately and efficiently
• Provide staff mentorship and in-service education
• Recommend and educate on assistive devices
• Uphold Medicare and state/federal documentation standards
• Participate in developing new rehabilitation programs and initiatives
• Maintain a clean, safe clinical environment and ensure compliance with infection control protocols
• Supervise assistant-level staff as needed
