Responsibilities:

• Provides home health nursing care to patients in assigned zip codes.
  
• Performs start of care visits and revisits with compliance to protocols.
  
• Documents patient assessments and updates care plans accordingly.
  
• Collaborates with healthcare team to ensure quality patient care.
  
• Adheres to all state and federal regulations regarding home health services.
