PRN Speech-Language Pathologist (SLP) | Crescent Manor Care Centers

Crescent Manor Care Centers is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team in Bennington, VT on a PRN basis .

As a Speech-Language Pathologist, you will:

• Assess residents who require speech, language, cognitive, or swallowing rehabilitation services

• Provide direct patient care based on each resident’s medical condition, functional abilities, limitations, and rehabilitation potential

• Develop and implement individualized treatment programs with realistic and achievable goals

• Monitor resident progress and modify treatment plans as appropriate

• Complete evaluations, progress notes, recertifications, and discharge documentation

• Educate residents, families, and staff on appropriate therapeutic techniques and strategies

• Collaborate with nursing, therapy, and other interdisciplinary team members to provide comprehensive, resident-centered care

• Maintain accurate and timely documentation

• Ensure compliance with confidentiality, HIPAA, Residents’ Rights, and applicable state and federal regulations

• Perform other duties as assigned by the Director of Rehabilitation
  

• Graduate of an accredited Speech-Language Pathology program

• Current and valid Vermont SLP license in good standing

• Strong communication, organizational, and interpersonal skills

• Experience in long-term care and/or geriatric care preferred, but not required

• Compassionate, dependable, and committed to providing high-quality resident care

• Ability to work effectively as part of an interdisciplinary team
