Home Hospice Registered Nurse

Schedule

AM shifts, 5 × 8-hour shifts per week, totaling 40 hours.

Job Description

We are seeking an experienced Home Hospice Registered Nurse with expertise in providing care in a home health setting. The role involves managing patient care, conducting assessments, and ensuring compliance with hospice protocols.

Responsibilities

• Conduct patient assessments and develop care plans.
• Administer medications and provide necessary interventions.
• Coordinate with interdisciplinary team members to ensure comprehensive care.
• Document care activities and updates in the patient management system.
• Maintain compliance with regulatory standards and protocols.

Required Experience / Certifications / Licensure

• Current RN license.
• Previous hospice experience is required.
• Experience with HomeCare HomeBase (HCHB) is required; candidates without this experience may be considered at a reduced rate with training from management.
• Valid driver's license and auto insurance are required.

Location

Coverage area includes Hemet, Menifee, and Murrieta, with potential for assignments within a 60-mile radius.
