Case Manager, New Hamshire

Our family of services comprises three distinct mental health treatment programs, including a specialized program for US Veterans. With over 15 unique locations, we manage and support 500+ dedicated employees serving over 300 residential clients daily located across five states: California, Idaho, Maine, New Hampshire, and Texas. As the Ridge Case Manager ,you willcollaboratewith other members of the clinical team,toserveas a liaison for the treatment team, client,familiesandaftercare services. Key Responsibilities: Provide comprehensive case management, including assessment, care planning, and ongoing support. Participate in the interdisciplinarytreatmentplanning process and perform proactive discharge planning. Provide education toclients and their familiesregardingcommunity resources,benefits, and coverage. Coordinate andfacilitatemeetings between theclients/families/residential care team.You will assistclientsand families in making difficult decisions while honoring their preferences and values. Facilitate group sessions and support recovery-focused programming. Ability to successfully document all clinical contacts and documentation through ElectronicMedicalSystem. Support a safe, structured, and trauma-informed residential environment. Facilitate and providetimelydocumentation. Adhering to company policies and proceduresregardingconfidentiality and privacy. Other duties may be assigned. Qualifications: Bachelor's degree in social work, psychology, healthcare administration, ora relatedfield (preferred). Minimum2years of experience in a residential or therapeutic setting. Leadership skills that foster a culture of inclusivity and excellence. Excellent communication, leadership, and organizational skills. Strong crisis management and conflict resolution skills. Salary Range

$28 - $30 USD
