Position: Social Worker Location: Honolulu, HI Pay: $84,00 to $85,000/year Experience: Previous experience working in a nursing home or long-term care setting. Education: Bachelor’s or Master’s degree in Social Work or a healthcare-related field. Type: Full-time; Direct Hire Schedule: Monday – Friday, 8:00 AM to 4:30 PM Kahu Malama Nurses is seeking a Social Worker to join a growing and dynamic team! Job Description Conduct assessments to identify residents' psychosocial needs. Develop individualized care plans in collaboration with residents, families, and healthcare providers. Provide counseling and support to residents and families to address emotional and social issues. Advocate for residents' rights and coordinate with external agencies as needed. Maintain accurate and timely documentation of services provided. Participate in care conferences and multidisciplinary team meetings. Collaborate with healthcare professionals to ensure holistic care. Investigate grievances and alleged abuses. Assist with admission and discharge planning. Position Requirements Proficiency with database software, Microsoft Office products, and office equipment. Strong communication and interpersonal skills. Ability to work collaboratively with a multidisciplinary team. Knowledge of social service resources and agencies.
