Consistent full‑time schedule Competitive compensation and comprehensive benefits package Medical, dental, and vision coverage Paid time off and paid holidays Professional development and continuing education opportunities Supportive and collaborative team environment Career growth opportunities within Rehab Without Walls What You'll Do Provide psychosocial assessments and behavioral health support for patients and families Develop individualized treatment and support plans based on patient needs and goals Deliver counseling, education, and resource coordination services Assist patients and families in navigating community resources and support systems Collaborate with physicians, therapists, case managers, and other treatment team members Advocate for patients throughout their rehabilitation journey Support emotional adjustment, coping, and successful community reintegration What We're Looking For Master’s degree in Social Work Current LCSW licensure in Oregon Strong communication, counseling, and care coordination skills Experience working with neurological, rehabilitation, behavioral health, or medical populations preferred Passion for helping individuals achieve meaningful outcomes and improved quality of life Responsibilities Perform comprehensive evaluations, assessing the social, emotional, and support needs of patients and families Set measurable objectives formulated with the rehab team Assist physicians and other health team members in understanding social and emotional factors related to patient health problems Instruct patients and families in coping with social and emotional responses to illness Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs Help patients/families utilize appropriate community resources to achieve objectives Qualifications Master’s degree in Clinical Social Work (LCSW) Current Licensed Clinical Social Worker licensure in the state of practice Minimum of one year experience in a rehabilitation setting with traumatic brain injury, spinal cord injury, or acute diagnosis CPR certification required Ability to lift 50 pounds Capacity for lifting or transferring patients, including up to 100 pounds, following safety procedures Requires fine motor skills, visual acuity, and the ability to stand or walk for extended periods Salary USD $37.64 / Hour #J-18808-Ljbffr Rehab Without Walls® Neuro Rehabilitation
