Clinical Staff Pharmacist - Pharmacy - FT - Variable

Job Details Position: Full time. Shift: Variable, less than 12 hours. Hours per week: 40. Exemption status: Non-Exempt. Job Summary The Clinical Staff Pharmacist at SVH provides comprehensive pharmaceutical care to patients by optimizing medication therapy, providing disease state prevention and management, and promoting health and wellness. This mission is accomplished by upholding the American College of Clinical Pharmacist philosophy of pharmaceutical care, blending a caring orientation with specialized therapeutic knowledge, experience, and judgment to ensure optimal patient outcomes. The role also contributes to the generation of new knowledge that advances health and quality of life, offering services to patients from birth to death. Education Qualifications Bachelor’s Degree of Science in Pharmacy from an accredited school of pharmacy. Required or Doctorate PharmD from an accredited school of pharmacy. Experience Qualifications 1 year Pharmacy practice experience as a licensed Pharmacist or successful completion of a Post Graduate-Year 1 accredited pharmacy practice residency. Preferred. Skills and Abilities Medical terminology and knowledge of medication names and how to effectively run a pharmacy. Ability to apply mathematical concepts such as exponents, logarithms, quadratic equations, and permutations to tasks like frequency distribution, determination of test reliability and validity, analysis of variance, correlation techniques, sampling theory, and factor analysis. Ability to define problems, collect data, establish facts, and draw valid conclusions. Interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Proficiency with spreadsheets, internet/e‑mail, database software, contact management, and electronic medical records. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Write reports, business correspondence, and procedure manuals. Present information effectively and respond to questions from managers, clients, customers, and the general public. Excellent customer service skills. Licenses and Certifications Pharmacist – KBP required within 90 days of hire. Basic Life Support – BLS required within 90 days of hire. Advanced Cardiovascular Life Support – ACLS required within 90 days of hire. Responsibilities Review, interpret, and verify provider medication orders, making recommendations and clinical interventions based on patient condition, disease states, age, and other factors such as safety and cost effectiveness. Complete pharmacy consults proficiently following pharmacy policies and procedures. Provide drug information to healthcare professionals and educate patients using interpersonal skills to communicate and interact with the healthcare team while multitasking to ensure completion of all assigned tasks. Facilitate completion of pharmacy medication distribution functions, directing pharmacy technician workflow and verifying accuracy of all final products prior to dispensing to patients. Manage medication use including compounding, preparation, selection, dispensing, storage, and stocking for the entire organization. Collaborate with the pharmacy team to ensure optimal patient care, maintaining positive relationships with coworkers and demonstrating social awareness. Educate and train pharmacy personnel and other healthcare professionals, serving as preceptors for pharmacy students and residents. Develop and conduct medication use evaluations as needed, reporting results to relevant committees or subcommittees to implement improvements in daily practice. Design, implement, and lead pharmacy process improvement initiatives, including those focused on evidence-based care, safety, financial sustainability, or quality improvement processes. Address and complete provider consults, provide drug information to healthcare professionals, and educate patients.
