Responsibilities

• Assess patients and develop individualized physical therapy plans.

• Implement evidence-based therapy techniques to improve mobility and function.

• Monitor patient progress and adjust plans to meet goals.

• Meet or exceed individual targets using outcome metrics and pipelines.

• Document treatment sessions and outcomes in electronic health records.

• Educate patients and families on home exercise programs and injury prevention.

• Collaborate with multidisciplinary team to optimize patient care.

• Maintain high quality of care while adhering to ethical and regulatory standards.

• Participate in professional development activities to advance skills.

• Assist in maintaining clinic workflow and ensure timely patient throughput.

• Provide feedback on process improvements and contribute to pipeline forecasting.

Experience/Qualifications

• Licensed Physical Therapist with active state license.

• Develop and adjust evidence-based therapy plans per patient goals.

• Demonstrate measurable progress via outcome metrics and pipelines.

• Commitment to ethical standards and regulatory compliance.

• Proficiency in documenting in electronic health records (EHR).

• Excellent communication for patient/family education and team collaboration.

• Ability to manage time and throughput to maintain clinic workflow.

• Collaborate with multidisciplinary team to optimize patient care.

• Participation in professional development and ongoing skills enhancement.

• Adaptability to a growth-focused, target-driven environment.
