Youth Case Manager II - Community-Based Housing & Stability
The Connection
The Connection is seeking a dedicated Case Manager 2 in Bridgeport, CT to provide home- and community-based case management services for homeless young adults. The role includes conducting intake assessments, assisting with housing, and collaborating with providers to help clients achieve stability. This position requires an Associate's Degree and at least 2 years of experience in case management.
