What You’ll Do:
- Conduct nutrition assessments, consultations, and follow-up evaluations.
- Develop and implement individualized nutrition care plans.
- Monitor nutritional status and recommend appropriate interventions.
- Collaborate with healthcare professionals to improve nutrition outcomes.
- Provide nutrition education and counseling as appropriate.
- Utilize the Nutrition Care Process to deliver high-quality nutrition care.
- Help improve patient outcomes using “ Food as Medicine ”.
Who Should Apply:
- Registered Dietitian Nutritionist (RDN) or Licensed Dietitian (LDN) .
