Qualifications:

• MD or DO from an accreditedmedical school
• Active Arizona medicallicense (or eligibility to obtain)
• Current DEA registration
• Board Certified or BoardEligible in Family Medicine or Internal Medicine
• Experience caring forgeriatric, post-acute care, SNF, LTC, or medically complex patient populationsis preferred
• Excellent clinical judgment,communication, and team collaboration skills
• Commitment to deliveringcompassionate, patient-centered care
