Job Summary We are offering a five thousand dollar sign‑on bonus! We are seeking a full‑time (32‑40 hours per week) Nurse Practitioner for our Hilltop Center located in Hilltop, WV. Responsibilities Reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. Collaborates on‑site with the Center Leadership Team for center specific goals and priorities. Collaborates with the Medical Director and/or Attending Physician for patient specific medical care. Provides primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center. Works collaboratively with Center Leadership to achieve AlignMed Partners mission of improving the lives we touch through the delivery of high quality health care and everyday compassion. Qualifications Graduate from an accredited School of Nursing program. Licensed as a Registered Nurse. Certified as a Nurse Practitioner with a Master’s Degree in Nursing and a concentration and specialty track in Older Adult and/or Family Nurse Practitioner (FNP). Minimum of two years of experience working with adult or geriatric population. APRN with an unrestricted professional license and DEA, for each state in which they practice.
