In-Home Nurse Practitioner: Geriatric & Telehealth

HarmonyCares is seeking a Nurse Practitioner to deliver annual risk assessments in patients' homes or via telehealth, within the scope of practice as delegated by the Collaborative Physician. You will conduct in-home assessments, document findings to NCQA standards, take histories, determine diagnoses, and coordinate care with PCPs and care teams. A valid NP license, board certification, BLS, and strong EHR skills are required; driving to patient locations is expected. #J-18808-Ljbffr HarmonyCares
