Supervising Care Manager

Under direct supervision of the Care Management Services Program or Project Director, the SCM is responsible for planning, organizing, developing, and implementing the principles and techniques employed in a comprehensive care management model for the assigned program’s target audience. ESSENTIAL DUTIES Supervising Care Management staff which may include Care Managers, Care Manager Aids, Nurse Care Managers, Interns, Community Health Workers, Care Management support staff and/or other Care Management staff or volunteers completing program requirements, providing training, and/or creating and implementing work performance improvement plans. Assigning caseloads, documenting member status, encounters, contracts, and other metrics in various websites, platforms, portals, and folders and tracking key performance indicators; reviewing performance with staff and updating Program Director a minimum of twice monthly. Coordinating with Intern Field Supervisor and college/university Field Liaison to train, coach, and oversee program interns; including participating in weekly intern supervision. Continuously evaluating program operations to assure delivery of program services as required within the CHC and the community and identifying and recommending changes to improve service and accessibility for program participants. Establishing and maintaining effective working relationships and conducting outreach with community partners providing services to clients, including attending community, provider, and State-level meetings/hearings. Coordinating with Enhanced Care Management and Community Health Worker Program Directors a minimum of monthly to develop, implement and enhance integration across service lines. Supervising the implementation and on-going development of case management, care coordination, and advocacy services for program participants and recommending updates to policies and procedures. Providing and/or assuring completion of on-going and periodic in-service training for care management staff, including, but not limited to: HIPAA, documentation, peer reviews, policies and procedures, universal precautions, safety in the field, etc. Conducting monthly staff meetings in coordination with Program Director. Resolving vendor/provider/participant issues as needed; may include attending State Hearing meetings. Functioning as part of an interdisciplinary and integrated organizational team. Recommending policy, outreach, and staff changes. Attending and representing the program in meetings with committees, community agencies, partners, and other related entities, and facilitating interagency linkages. Attending events that may take place on weekends or after traditional work hours. Other duties as assigned. REQUIRED DUTIES Oversight of all care management program staff may be required, including administrative staff, social work and nurse care managers, interns and volunteers. Providing detailed program metrics to the Program Director. Coordinating with MSSP Interns Field Instructors with weekly intern supervision. Engaging in outreach events, which may include some weekends and evenings. Participation in community and provider meetings and actively representing Care Management Program (CHC, MSSP, ECM) and developing working relationships with providers. Participating in coordinating with ECM and CHW Program Directors monthly to develop, implement, sustain and enhance integration across programs. Participating in providing ongoing in-service trainings for Care Management Program staff Assuring daily office coverage in collaboration with Director to assure equitable coverage. Travel to all CHC MSSP counties of service when needed. QUALIFICATION REQUIREMENTS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION AND/OR EXPERIENCE Master’s degree in administration, community health, public health, nursing, gerontology, social work, or a related field, and a minimum of three years’ experience of responsible managerial or administrative roles; preferably in the social service field strongly preferred; or four years’ experience and a bachelor’s degree in stated fields may be substituted for a master’s degree. CCM, LCSW, or LMFT strongly preferred. Ability to lead a diverse, interdisciplinary team; 2 years’ experience preferred. Requires a high degree of professionalism, and willingness and ability to effectively collaborate with community partners, peers, and oversight agencies. Strong verbal and written communication skills. Proficient technical knowledge with current/prior versions of Microsoft and Apple operating systems, Office applications, Outlook/Exchange, and Microsoft SharePoint. Demonstrated understanding of and genuine commitment to the diverse populations we serve, as evidenced through relevant lived or professional experience. LANGUAGE SKILLS Fluent in English with good communication and writing skills, bilingual preferred. REASONING ABILITY Ability to use good judgment to conduct professional and collaborative activities and solve practical problems within established limits and guidelines. Ability to recognize instances in which consultation with Director(s), Fiscal Department, and/or Senior Director should be sought. TRAVELING REQUIREMENTS Have reliable form of transportation and insurance coverage. Must be willing and able to travel to all service areas including Alpine, Colusa, El Dorado, Placer, Sacramento, Sutter, Yolo, and Yuba counties. WORK LOCATION AND HOURS In office Monday – Friday from 8:30 am – 5:00 pm. Occasionally weekend and evening if there are meetings or outreach events.
