Maybelle Center is seeking a Direct Caregiver to provide trauma-informed, person-centered care that supports residents in completing activities of daily living (ADLs), promoting behavioral health stabilization, substance use recovery, and overall wellness in a behavioral health assisted living setting. This role supports the implementation of the resident's Service Plan (SP), Temporary Service Plan (TSP), and Behavior Support Plan (BSP) while maintaining compliance with Oregon Administrative Rules (OAR 411-054).
As the Direct Caregiver, you will not only be working as part of a cohesive and interdisciplinary resident care team, but you will also be supporting nursing services in their ongoing endeavor to deliver comprehensive, holistic health models. You will actively collaborate with team members, external partners, and residents to promote recovery and stability in a respectful and culturally sensitive manner.

- On-site at our assisted living facility (Macdonald Residence)
- Day Shift and Floater(s)
- As a floater, you must have flexibility in your schedule to cover two of the following shifts:
- DAY (6 AM - 2:30 PM)
- Swing (2 PM - 10:30 PM)
- NOC (10 PM - 6:30 AM)

- Promotes a trauma-informed care approach by creating a safe, supportive, and understanding environment for residents.
- Supports residents in maintaining their independence and autonomy, respecting their preferences and individuality.
- Fosters a homelike and inclusive atmosphere within the community.
- Follows the individual resident Service Plan and provides assistance with ADLs, including bathing, grooming, toileting, dressing, hygiene, and mobility, with awareness of and sensitivity to behavioral needs.
- Provides interventions as outlined in the Behavior Support Plan.
- Observes and monitors residents for changes in condition, including physical, emotional, cognitive, and behavioral changes, and reports to RN Case Manager and/or appropriate supervisor.
- Read and follow Temporary Service Plans for each resident daily.
- Provide feedback on the efficacy of Behavior Support Plans and Service Plans.
- Assist residents with organizing their living spaces and perform light housekeeping tasks, including cleaning biohazards.
- Measures and documents vital signs, weights, and other health indicators as assigned and trained.
- Collects, completes, and ensures proper labeling and storage of specimens for laboratory studies (UA) when designated by the RN Case Manager, LPN, or DNS.
- Demonstrates understanding of and adheres to infection control procedures, including PPE use, hand hygiene, standard and isolation precautions.
- Follows community emergency protocols for fire, fall, missing resident, behavioral crisis, medical emergencies, overdose, substance use, and disaster drills.
- Identifies and documents environmental hazards, maintenance issues, and safety concerns in the maintenance log.
- Assist with implementing de-escalation strategies under the guidance of the Behavioral Health team.
- Attends and actively participates in shift huddles, scheduled trainings, in-services, and team meetings.
- Collaborates with Direct Care Lead and Direct Care Float for coverage during huddle and communicates huddle meeting minutes afterwards.
- Coordinates with Direct Care Lead and Direct Care Float for lunch coverage.
- Contributes to interdisciplinary collaboration and shared decision-making.
- Accepts feedback and engages in continuous learning to improve care delivery, communication, and crisis response.
- Provides support during peak times, emergencies, or when residents require additional care.
- Observes and reports maintenance or safety concerns in resident units or shared areas.
- Documents all services provided, resident interactions, and observations daily in PointClickCare and paper documentation, including entries related to SP, BSP, IRs, and TSP implementation.
- Completes or assists in completing incident reports when involved or when the first responder witnesses an incident, and follows up with supervisory staff, per policy.
- Provide consultation and maintain effective communication with County representatives and other external agencies.
- Facilitate treatment and continuity of care by collaborating with community partners, legal representatives, guardians, and other stakeholders.
- Attends all required education, training, and competency re-evaluation sessions.
- Perform other tasks, within scope, as directed by the Direct Care Manager, RN Case Manager, LPN, Director of Behavior Services, or Director of Nursing Services.
- Fosters a respectful, inclusive, and caring atmosphere for residents, families, and team members.
- Maintains confidentiality and privacy of all residents, employees, and organizational information.
- Contributes to a cohesive, team-oriented work environment.
- Practices safe working habits and contributes to a secure work setting.
- Actively participates in ongoing training, in-services, and scheduled meetings.
- Supports quality assurance performance improvement projects and performs other duties as assigned within scope and role.

- Mandatory completion and maintenance of certifications and continued education required by Oregon Administrative Rules (OAR), Oregon DHS, and internal policy.
- Required to wear an assigned name tag and carry designated safety and communication tools (e.g., community keys, iAlert device, walkie-talkie, gait belt, closed-toe shoes, scrub pants, and company-provided shirts) while on duty.
- Must have a high school diploma or general education degree.
- Working knowledge of equipment, supplies, and chemicals used in clinical and housing operations.
- Food Handler's card required.
