About the Role:

As an Occupational Therapist, you will be responsible for creating customized therapy plans to help our residents achieve their functional and cognitive goals. With a strong focus on individualized care, you will work closely with our residents, families, and interdisciplinary team to design and implement evidence-based interventions that promote independence, mobility, and overall well-being. Your expertise will help our residents regain strength, confidence, and a sense of purpose.

Responsibilities:

• Develop and implement comprehensive occupational therapy plans that address the unique needs of each resident
• Conduct initial and ongoing assessments to evaluate resident progress and adjust treatment plans as needed
• Design and implement evidence-based interventions to improve physical, cognitive, and emotional function
• Collaborate with our interdisciplinary team, including nursing, physical therapy, and speech therapy, to ensure comprehensive care and individualized goal development
• Maintain accurate and detailed records of resident progress, treatment plans, and therapy sessions
• Participate in in-service training and education to stay current with best practices and industry developments
• Provide education and support to residents, families, and caregivers to promote independence and self-advocacy
