In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Worcester, MA

Part-Time Clinician (Nurse Practitioner or Physician Assistant) Signify Health, part of CVS Health, is seeking a Part Time Clinician to provide in‑home health evaluations. The role involves engaging with patients in their homes and delivering comprehensive, personalized health assessments. Key Responsibilities Conduct approximately 45‑minute in‑home visits that include medication and medical history review, physical evaluation, and, if ordered, diagnostic tests (e.g., diabetic eye exams, spirometry). Use an iPad and clinical workflow tools; you are not required to prescribe medicine, order lab tests, or change patients' existing treatment regimens. Participate in virtual visits as needed. Provide all Signify Health services including diagnostic and preventive services products (spirometry, DEE, bone density screening, etc.). Be flexible to travel locally and within licensed states; cover Worcester and surrounding area. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses preferred. Benefits and Compensation Part‑time employees may receive selected benefits, including malpractice insurance coverage, supplies and other perks, and pay per completed Diagnostic and Preventive Services (DPS) . Anticipated weekly hours: 29.5. Base pay per in‑home evaluation: $46.03 – $99.14 per hour (or base annual salary for full‑time equivalents). Pay depends on experience, education, geography, and other factors.
