Position Purpose Under the supervision of the Director of Therapy, the Physical Therapist evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. The therapist establishes treatment plans and uses a variety of methods, including exercises, stretching maneuvers, hands‑on therapy, and equipment to ease pain and increase mobility. Supervision of physical therapy assistants within Oklahoma State requirements is also required. Organization The position reports to the Director of Therapy. Essential Functions Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function. Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes. Provide a written evaluation and plan of care defining physical therapy needs, goals, progress, and expected outcomes. Deliver physical therapy services in compliance with prescribed physician orders. Maintain plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determining discharge care plans as appropriate. Coordinate discharge planning, including collaborating and educating the patient and caretakers on home exercise plans. Participate in coordination of services with the health care team, physicians, parents/guardians, nurses, etc. Document according to policy and procedure, requiring minimal supervision or instruction. Ensure documentation results in no technical or clinical denials after payer review. Follow policies and procedures and conform to all applicable patient care standards, licensure regulations, and state laws. Maintain effective communication between staff, health care team members, and family. Serve as a resource consultant for clinical staff. Supervise other personnel (home health aide, physical therapy assistant) as applicable. Complete OASIS assessments where allowed by state professional practice and regulation. Perform other duties as assigned. Knowledge & Experience Degree in Physical Therapy from an accredited university/program. Current, unrestricted state license as a Physical Therapist in Oklahoma. Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedure. Current Basic Cardiac Life Support (BCLS) certification required. 1–2 years of recent Home Health experience preferred. Active Oklahoma driver’s license. Current vehicle insurance and registration. Required vehicle ownership. Medical knowledge, patience, compassion, kindness, critical thinking skills, knowledge of infection control practices, stamina, physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, and empathy. Physical Requirements The physical demands of this position include moderately heavy exertion such as extensive driving, assisting patients in transfer activities (wheelchair, bed, tub, commode), and providing substantial support to individuals in ambulation. Employees must regularly use hands, fingers, and speak or hear, stand, walk, reach, stoop, kneel, crouch, or crawl, and occasionally sit. Lifting and/or moving over 100 pounds may be required. Vision requirements include close vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Working Conditions General office setting, patient home settings, exposure to infectious diseases, automobile, and security and privacy training will be provided.
